www.phs.org/centennialcare 3 Presbyterian has a Baby Bonus Program that offers rewards for taking your baby to well-baby appointments within the first 30 months of life. We want to help you keep your baby healthy. Babies need six well-baby checkups in the first 15 months of life, with two more visits before they are 30 months old. Presbyterian will provide gift card incentives for up to eight well-baby visits. The CHW team lives and works in New Mexico communities, and they are trained to help our members find resources within their community so they stay healthy and receive the support they need. CHWs can help members: ✔ Locate food ✔ Find housing ✔ Pay utilities ✔ Locate support during pregnancy ✔ Manage their health ✔ Find a new provider The CHW team also supports members with needs related to asthma, diabetes, behavioral health, and addiction. Many team members live in or near Tribal communities. This helps them stay informed of Native American members’ background, culture, language, and needs. They also work with local Community Health Representative programs so they can better meet our members’ needs. Thank you for helping our youngest members receive the care they need when they need it.
