One of the ways we do this is through care coordination. Care coordination is a service that Presbyterian offers for members with chronic long-term, complex, or behavioral health needs. The level of care coordination is based on the member’s healthcare needs. Care coordinators work with the member, their family or support system, and their provider to help members get the care they need when they need it. Care coordinators live and work around the state of New Mexico. They learn and respect our members’ communities, cultures, and beliefs, and they work with them to create goals that meet their needs. Care coordinators also help members: ● Coordinate a discharge to home if they are in a hospital, skilled nursing facility, or rehabilitation center ● Find medical, behavioral, and social services in their area ● Get the covered services they need ● Stay in their home and live on their own as much as possible Care coordinators also speak with the member’s provider to make sure they are getting the correct medication. They can also refer members to Community Health Workers or Community Health Representatives for help.
