2 We know that customs are an important part of health and wellness for Native American people. We seek to honor and support your culture with the Traditional Medicine value-added service. This value-added service can help you with $300 per calendar year (January 1 to December 31) to cover costs for Native American traditional or ceremonial services. This service is only available to Native American Centennial Care members and is to be used for traditional medicine or ceremonial purposes only. Your Native American status must be shown on your PresID membership card. If your membership card does not show Centennial Care Native American, please contact your local Income Support Division office to make the needed changes. We have recently updated the application to make it easier for you! You can get the new application by: ● Calling the Presbyterian Customer Service Center (PCSC) at (505) 923-5200 or 1-888-977-2333 ● Calling the Native American Affairs program at (505) 228-1191 ● Downloading the applications at: www. phs.org/health-plans/centennial-care- medicaid/Pages/native-american- communities.aspx Traditional Medicine: A value-added service available to you! Online application coming soon! Starting in fall 2021, Native American members will be able to fill out and submit the Traditional Medicine value-added service form online. This will make it easier for members to access the form and send it to Presbyterian. This will also help Presbyterian speed up its review process. The application form must be filled out completely, including signature, current address, date, and your Presbyterian member ID number. Incomplete forms cannot be processed.
