We know that customs are a vital part of health and wellness for Native American people. We seek to honor and support your culture with the Traditional Medicine Benefit. As a value-added benefit, we can help you with $300 per calendar year (January 1 to December 31) to help pay for traditional or ceremonial services. This benefit is available to Centennial Care Native American members only. Your Native American status must be shown on your Pres ID membership card. If your membership card does not show Centennial Care Native American, please contact your local Income Support (ISD) office to make the needed changes. You can also get an application by calling the Presbyterian Customer Service Center (PCSC) at (505) 923-5200 or 1-888-977-2333 . You can also contact the Native American Affairs program at (505) 923-8438 . Instructions on how to fill out and submit the application are given on the form. Please fill out all the information, including signature, current address, date, and your Presbyterian member ID number. Please allow five to six weeks for processing. Track your blood sugar ● ● Check your levels two to four times a day. ● ● Get an A1C screening regularly. The A1C shows your average blood sugar level over the past three months. 2. Healthy eating and drinking ● ● Eat every four to five hours. ● ● Drink water. ● ● Limit caffeinated and sugary drinks. 3. Move your body ● ● Exercise daily, at least 10 minutes at a time. 4. Take diabetes medicine (if prescribed) ● ● Know when, how, and why to take your medicine. 5. Visit your provider (at least twice a year) ● ● Go over your diabetes visit checklist. ● ● Talk about diabetes screening. Taking care of your diabetes Having diabetes means you have too much sugar (glucose) in your blood. High amounts of sugar in the blood can harm your body. Keeping blood sugar close to healthy levels can prevent serious health issues. Healthy blood sugar levels BEFORE eating a meal: Between 80–130 One to two hours AFTER eating a meal: Less than 180 Your range may be different if you have other health conditions or if your blood sugar is often too high or too low. Always follow your provider’s recommendations.
