6 Advance directives You may wonder how to make sure your family knows what your wishes are if you are injured or sick and can’t speak for yourself. An advance directive helps to make your wishes clear so that they are followed. Your advance directive may include: ● ● Choosing a healthcare decision-maker. This is the person(s) who will speak about your medical decisions for you if you are injured or sick and cannot speak for yourself. ● ● Information about your healthcare decisions. Your advance directive can include specific orders about what medical support you want or do not want if you become seriously ill. ● ● A do-not-resuscitate order. Sometimes this is called a DNR. A DNR order says that you do not want cardiopulmonary resuscitation (CPR or mouth- to-mouth) to be performed if your heart stops beating or if you stop breathing. You do not need to use a specific form, but Presbyterian has an advance directive form that you may use. You do not need a lawyer to make an advance directive. You also do not need a notary or witness to sign your advance directive in New Mexico. Each state may have different guidelines. If you have an advance directive, give a copy to your provider to add to your medical record. You can learn more about advance directives by calling the Presbyterian Customer Service Center at the number on your member ID card. Information is also in your Member Handbook. PresRN Presbyterian Health Plan members have access to PresRN, a nurse advice line available to you 24 hours a day, seven days a week (including holidays). There is no cost to call our registered nurses (RN) for answers to your questions and health concerns. As always, if you are having a medical emergency, please call 911 . What is PresRN? PresRN is an easy way to speak with a Presbyterian nurse if you are not feeling well and do not know what to do. Just call (505) 923-5677 or 1-888-730-2300 and one of our qualified nurses will listen to your health concerns, assess your symptoms, and give you the answers that you need to care for you and your family. Our nurses are happy to answer general health questions when you are healthy too. For more information on PresRN, go to www.phs.org and type PresRN in the search field. myPRES With myPRES you have secure online access to manage and get information about your health. Through myPRES you can: • Check how much of your deductible you have met. • Review your claims history and Explanation of Benefits (EOBs). • Print a temporary ID card and request a replacement. • Manage your prescriptions (drugs), find pharmacies (drugstores), check your drug claim history, and more. • Use MyChart—patients of Presbyterian Medical Group can view parts of their electronic health record (EMR). You can do things like: » Get test and lab results » View drug and allergy information » Schedule an appointment (visit) » View records of past visits » Send messages to your care team » Renew a prescription (drug) Visit www.phs.org/mypres to sign up today!
