8 Centennial Care member benefits Presbyterian Centennial Care provides a full range of covered benefits. These include physical, behavioral and long-term care services and supports. Some of these benefits include: Baby Benefits Presbyterian offers a program that can provide you with tips for a healthy pregnancy and baby. You can enroll in the Presbyterian Baby Benefits prenatal program in one of these ways: ● ● Online at mypres.phs.org ● ● Print enrollment form at www.phs.org/ CentennialCareBabybenefits ● ● Call us at (505) 923-5017 ● ● Mail your enrollment certificate: Presbyterian Health Plan, P.O. Box 27489 Albuquerque, NM 87125 You can earn rewards by completing the following: Step 1: Have your first prenatal (pre-birth) visit before week 13 of your pregnancy and earn a $25 gift card. Step 2: Go to at least 10 (or 80 percent) of your recommended visits and earn a $75 gift card. Step 3: Have a visit for your health within three to eight weeks (21–56 days) after you baby is born (postpartum) and earn a $50 gift card. Assured Imaging mobile events There are many ways to stay healthy. One of those ways is to have recommended screenings (tests) completed. Presbyterian will reward you with a $25 dollar gift card for completing your recommended screenings at an Assured Imaging event. Call Assured Imaging at 1-888-233-6121 to schedule your screening. Here are some of the screenings that you can do with Assured Imaging: ● ● Mammogram (breast x-ray) ● ● Bone density evaluation (DEXA) ● ● Diabetic screening { { Retinopathy eye screening { { A1C { { Nephropathy (kidney screening) ● ● Colorectal cancer screening Transportation benefits Presbyterian Centennial Care provides non-emergency transportation to covered medical and behavioral health services. Our transportation provider has different types of transportation available. The type you qualify for is based on your medical need, your pickup location, and where you are going.
