www.phs.org/centennialcare 11 As a Centennial Care member, Presbyterian offers you transportation services to receive covered services. Our contracted transportation vendor is Superior Medical Transportation (SMT). They can be reached directly by calling 1-855-774-7737 . We want to let you know that there has been an update to the member reimbursement policy, starting Sept. 1, 2018 . Please follow the process below for member mileage reimbursement requests: ● ● Transportation is provided only to services that are covered by Presbyterian Centennial Care. Note, some pharmacy items are a covered benefit but transportation to a pharmacy is not covered. ● ● Transportation is only covered to the closest qualified provider. Call Presbyterian Customer Service Center to help you find the closest appropriate provider. ● ● You must schedule your transportation at least 48 hours before your scheduled appointment. ● ● Call SMT at 1-855-774-7737 to request transportation. SMT is our contracted transportation vendor. They can get you transportation based on your medical need, pickup location, and where you are going. ● ● Travel must be a minimum of 65 miles one-way per appointment to be considered for mileage reimbursement. ● ● We may ask for a letter from your provider telling us why your transportation reimbursement request is medically necessary. The letter will be reviewed for clinical appropriateness. ● ● All member mileage reimbursement requests must be approved before your appointment. If your mileage request does not meet the criteria listed above, your request will not be approved. When it comes to fat, salt, and sugar content, comfort foods can really take you out of your comfort zone. To make favorite foods healthier, try these tips: • For cheesy dishes, choose strong- tasting varieties—like Parmesan or sharp cheddar—so you can use less. • Try low-sodium and reduced-fat soups in creamy casseroles. • Make dips with nonfat Greek yogurt instead of mayo. • Swap regular french fries for sweet potato fries. • Instead of a chocolate bar, have chocolate-dipped strawberries. • Slip spinach into your favorite casseroles, mac and cheese, meatloaf, and pizza— and take added comfort in knowing you made them healthier! Phone calls During the year, you may receive outreach calls from Eliza on behalf of Presbyterian. The types of calls you can expect are: • Reminders for appointments, screenings, or health events • Education and health tips • Surveys about your health or the care you receive • Medication refill reminders Please be assured that your information is secure and confidential. Your participation helps us to better support your needs. Transition from pediatric to adult care When a Centennial Care member turns 21 or is pregnant, he or she may need care that is different from what they have been receiving. Presbyterian can help you to move from pediatric care (care for children) to adult primary care. This includes helping pregnant members find an obstetrician.
