7 www.phs.org/centennialcare Transportation Services Presbyterian contracts with Superior Medical Transportation (SMT) for nonemergent transportation for covered medical and behavioral health appointments. You must be an eligible Presbyterian Centennial Care member. WHAT INFORMATIONWILL I NEED WHEN CALLING SMT TO RESERVE A RIDE? You will need the following information when you call to schedule a ride: ■ ■ Your full name, date of birth, and Presbyterian Centennial Care Member identification (ID) number from your Presbyterian insurance card ■ ■ Time of the appointment ■ ■ Address of the provider or facility ■ ■ Name and phone number of the provider or facility ■ ■ A list of your special needs (medical equipment, oxygen, wheelchair, child car seat, qualified approved attendant, walker, etc.) ■ ■ Reason for the appointment (you are not required to provide the details of your appointment, only the type of appointment, such as primary care, dental, vision, or behavioral health) SMT will call your doctor or other healthcare provider to verify your appointment prior to your scheduled visit. PRESBYTERIAN’S BARIATRIC PROGRAM A team of Guardian Angels Making the choice to have bariatric surgery was not an easy one for Sandra. She had fear, shame, and embarrassment about being overweight. The thought of making a permanent change to her body was scary too. But the more Sandra learned about Presbyterian’s Bariatric program, the more confident she felt about the surgery. Her experience with the team was such a positive one that she wanted to give back. Sandra chose to honor William Rice, MD, and his team through the Guardian Angel Program. She praised Dr. Rice for his leadership of the program and support of the patients he helps through the surgery. She also showed her gratitude for each of the team members who cared for her during the process. Ashley Hansen, PA, walked Sandra through her options and helped her choose the best procedure. Trish Woolley, Financial Advocate (not pictured), helped calm Sandra’s fears about the cost of the program and made the financial part of the surgery one less thing to worry about. Cindy Chall-Silva, Nutritionist, helped Sandra understand the nutritional requirements that go along with the surgery. She also offered guidance without judgment as Sandra made changes to her habits. Holly Garcia, RN, was there for Sandra every step of the way and continues to provide help through the post-operative support groups offered to patients. Sandra’s gift to Presbyterian Healthcare Foundation’s Bariatric Fund will help provide resources for the bariatric team to continue their important work for patients.
