2 Healthcare easier with a care coordinator Presbyterian has a care coordination program for members. The program focuses on members who have special healthcare needs and those with complex health problems. Our teams are here to help you get the care you need. Care coordinators work with you to create a plan of care to meet your needs. Care coordination can: ■ ■ Help you with long-term illnesses and injuries ■ ■ Help you communicate with your healthcare team ■ ■ Make sure you have what you need when you go home from the hospital ■ ■ Help you and your caregiver(s) understand medications, diets, and other care needs ■ ■ Work with members who have mental or emotional needs, or drug or alcohol problems ■ ■ Help members who have special needs such as transplants, high-risk pregnancies, premature babies, and children who need special services or equipment For more details or for help with these services, please call our Presbyterian Customer Service Center at the number on the back of your ID card.
