All year, this online program helps you: • Build healthy habits • Lose weight and keep it off • Get more active • Sleep better and manage stress • Prevent or delay type 2 diabetes What’s included? • Your own lifestyle coach, a person who helps you build healthy habits • Access to your lifestyle coach by phone or online • Online group classes • An app that helps you choose healthy foods • A scale to track your weight and send information to your app Who is eligible? Presbyterian Centennial Care adult members with prediabetes or certain risk factors for type 2 diabetes. Find out if the program is right for you by calling 1-855-249-8587 . We’re committed to providing you access to high-quality care and service. If you’re not happy with our service, we want to know. Some problems call for formal attention. In those cases, you have a right to file a grievance, a complaint, or an appeal. ● ● Complaint (also referred to as grievance): If you have a complaint about care or services you receive, you can file a grievance. ● ● Appeal: If we deny a request for service or care and you do not agree, you can file an appeal. If you have a concern, the first thing to do is to contact the Presbyterian Customer Service Center (PCSC). The numbers are listed on your member ID card, in your Member Handbook, and in this newsletter. If the PCSC can’t resolve your concern, you can ask to speak to a Presbyterian Grievance Coordinator. You may also write to the Grievance Coordinator for written details about our procedures or your concerns. The program focuses on members who need special help with access to care for their healthcare needs or who have complex health problems. If you are one of those members, we have teams to help you get access to the care you need. Care coordinators work with you to create a plan of care to meet your needs. Care coordination can: • Help you get access to healthcare services you need • Help you with long-term illnesses and injuries • Make sure you have what you need when you go home from the hospital • Help with communications between you and your Healthcare Team • Help you and your caregiver(s) understand medications, diets, and other care needs • Help members who have special needs such as transplants, high-risk pregnancies, premature babies, and children who need special services or equipment • Work with members who have mental or emotional needs, or drug or alcohol problems • Help members with diabetes, chronic obstructive pulmonary disease (COPD), heart disease, and heart failure to manage their health For more details or for help with these services, please call the Presbyterian Customer Service Center at the number on the back of your ID card.
